How it Works
A clear, supportive path—from where you are to where you want to be.
Step 2
Intake & Assessment (60–90 minutes)
Your first session is a deep dive into your full picture.

We look at:

-
Your goals
-
Your daily routines
-
Your environment
-
Where things feel most stuck—or most missing
This is not a form or checklist.

It’s a conversation—and often includes seeing your actual space, which tells us far more than paperwork ever could.

By the end, you’ll have clarity on what we’re focusing on and what comes next.
Step 3
Sessions (Where the real work happens)
This is where Thrive OT is different.

We don’t give you a plan and send you home.
We work through it with you—in real time, using clinical reasoning to guide each step.

-
In your home
-
In your community
Sessions are designed to support real-life progress—not fit into a rigid time block.
Most sessions range from 1–2 hours, with the option to combine time for longer projects, home organization, or community outings.
Services are offered in flexible session packages rather than per-visit, allowing care to match your schedule, priorities, and pace.
Each package includes:

-
Pre-session planning and preparation
-
Personalized treatment plan
-
Session summaries with next steps
-
Accountability and check-ins as needed
-
Family collaboration and meetings
All clients begin with a comprehensive Initial Assessment to ensure services are aligned with your goals.
Step 4
Family Check-In (When helpful)
These are used to:

-
Share progress
-
Align on goals
-
Keep everyone on the same page

For clients with cognitive changes, family involvement is often part of the process, and we build a clear, sustainable plan together.
Step 5
Independence
The goal has always been your independence—not ongoing support.

As things begin to click:

-
Sessions become less frequent
-
Support gradually fades
-
Confidence takes over
We intentionally step back so that what you’ve built becomes your own.
And if life changes later, you can always come back—you won’t be starting from scratch.
